District Health Society Jobs: The applications should be addressed to the Chief Medical officer (Vice Chairman) of the District Health Society Rajouri on the prescribed format (Annexure-A). Interested candidates should apply on the format given as (Annexure-A), along with attested copies of supporting documents/certificates & Demand Draft of Rs.200/- payable in favor of Chief Medical Officer Rajouri, which must reach in the Office of the Chief Medical Officer Rajouri by hand against proper receipt within 21 days from date of publication of this notice.

Interested candidates should apply on the format given as (Annexure-A), along with attested copies of supporting documents/certificates & Demand Draft of Rs.200/- payable in favor of Chief Medical Officer Rajouri, which must reach in the Office of the Chief Medical Officer Rajouri by hand against proper receipt within 21 days from date of publication of this notice.

Initially the candidates shall be hired upto 31st March 2023 and further extendable on yearly basis upon the performance of the candidates/approval of the posts by the NHM

Date of Birth Certificate

Marks Certificate of 10th

Marks Certificate of 10+2

Basic/ essential qualification certificates.

Degree/ Diploma of requisite Qualification.

Marks sheet of all years/one consolidated marks sheet indicating marks of all the years of the Degree /Diploma.

Domicile certificate issued by Tehsildar concerned.

2 Passport size Photographs.
